- ABOUT US
About JACS Care
JACS Care is a specialist provider of Supported Living services for adults with Learning Disabilities. Our service is built upon the principles of dignity, respect, choice, and inclusion, ensuring that every individual remains at the centre of all decisions affecting their life.
We recognise that every person has unique strengths, talents, aspirations, and support needs. Our role is not simply to provide support, but to build positive relationships that encourage personal growth, confidence, and independence.
Our experienced team works collaboratively with families, advocates, social workers, healthcare professionals, and commissioners to ensure that support remains responsive, effective, and focused on long-term outcomes.
Empowering Independence. Enabling Opportunity. Transforming Lives.
At JACS Care, we believe that every individual deserves the opportunity to live a meaningful, fulfilling, and independent life within their community. Our Supported Living services are designed to empower adults with Learning Disabilities to develop confidence, build life skills, maintain relationships, and achieve their personal goals while receiving the right level of support.
We understand that independence looks different for everyone. For some people, it may mean learning to manage their own home, while for others it may involve accessing education, employment, volunteering opportunities, or becoming more active within their community. Whatever the goal, our focus remains the same: helping individuals achieve positive outcomes and live the life they choose.
Through person-centred support, specialist knowledge, and our Trinity Pathway Model, we create environments where people can thrive, grow, and realise their full potential.
The Trinity Pathway Model
The Trinity Pathway Model is the framework that underpins all support provided by JACS Care. It moves beyond traditional care models by focusing on whole-life development and ensuring that individuals are supported to achieve meaningful outcomes across every aspect of their lives.
Every support plan, review, activity programme, and personal goal is linked to one or more of the Trinity domains.
